What is EVV?

EVV stands for Electronic Visit Verification. It’s a system used to verify that home care services were provided as scheduled. It helps ensure that caregivers are delivering care accurately and on time.

EVV captures the following:

  • Date of the service

  • Time the caregiver arrives and leaves

  • Location of the service

  • Type of service provided

  • Name of the caregiver and participant

This process protects both the caregiver and the participant by improving transparency and accountability.

How Do we Track EVV?

At Lagom Care, we use AxisCare to track all EVV-required visits. AxisCare is our agency’s secure, mobile-friendly platform that makes clocking in, clocking out, and logging visit information simple and reliable.

Why EVV Matters

  • Compliance: EVV is required by law for Medicaid-funded services.

  • Accuracy: Ensures services are billed correctly and care is documented properly.

  • Accountability: Provides a digital record of visits, reducing errors and fraud.

  • Efficiency: Helps caregivers and agencies stay organized and connected.

New State Requirement: 85% EVV Compliance

Starting January 1, 2025, Pennsylvania is enforcing a new mandate that requires at least 85% of all Personal Care and Home Health Care Services to be properly verified through EVV.

This is not just a recommendation—it’s a mandatory federal and state regulation under the 21st Century Cures Act and applies to all Community HealthChoices-MCO providers.

What this means for caregivers and providers:

  • You must clock in and out using EVV for at least 85% of all your visits.

  • Falling below this threshold for three consecutive months may result in a Corrective Action Plan, additional training, or in severe cases, removal from the provider network.

  • Staying compliant ensures participants receive uninterrupted services and Lagom Care remains a trusted provider.

🕒 Availability:

  • Monday – Friday: 7 AM – 11 PM

  • Weekends: Available for 4 hours

 FAQs

  • If you forget to clock in or out, report it to the office immediately. You may be asked to complete a manual visit correction form. Repeated missed punches can affect compliance.

  • Yes, you can use your personal device as long as you have the AxisCare app installed and location services are enabled during clock-in and clock-out.

  • No. EVV only verifies your location at the time of clock-in and clock-out. It does not track you throughout the visit.

  • If you rely on a landline for EVV and it’s unavailable, call the office for instructions or use the mobile app as a backup if possible.

  • In the AxisCare app, you’ll see confirmation after syncing. If you're offline, the visit will sync automatically once you regain a connection.

  • You cannot edit a visit after it's been submitted. Contact the office if changes are needed, such as incorrect times or notes.

  • We understand mistakes happen. Just report any issues promptly. Our focus is on helping you succeed and maintain compliance.

  • Yes. Any visit that falls under Personal Care or Home Health services and is billable through Medicaid must be tracked via EVV, regardless of length.

  • You must clock in and out separately for each participant’s visit. Do not overlap or combine visits, even if they’re back-to-back.
